STARMARK

Starmark, which is fully insured by Trustmark Life Insurance Company, provides flexible healthcare benefits to employer groups with two to 50 lives. Starmark offers HSA- and HRA-compatible plans, PPOs and numerous ancillary products to build a strong plan portfolio.

GROUP MEDICAL

Starmark allows for the selection of the most appropriate PPO networks from a variety of options, including Premier Health Systems, Medcost, Health Care Savings and PHCS. Several plan designs are available: HSA Select, HRA Direct, PPO, PPO Advantage, Qualified High Deductible and Indemnity Plans. Maternity is optional on all plans with less than 15 employees, and an office visit feature may be selected on PPO, PPO Advantage and Indemnity Plans. Starmark's combined in- and out-of-network lifetime maximum is $5 million with the exeption of the Indemnity Plan, which is $2 million. The out-of-network lifetime maximum is $2 million for all plans.
